- have a disability and / or poor health including mental ill health
- ready and able to make positive changes to your life
Our purpose
Our overall purpose is to help you improve your health and wellbeing.
To do this, we take time to get to know you and understand what is important to you. Helping you to identify your skills, talents and passions including good life aspirations and goals.
We also want to understand the barriers to achieving these aspirations and goals, and the support you can draw upon from your personal resources and those around you.
We encourage and coach you to:
- take action on factors that influence your health and wellbeing
- become more independent and resilient
- engage with your local communities and social networks
What you can expect from us
Our first conversation will be to get some basic information such as:
- what you would like to change and what are your goals
- who is already involved in your life
- what support is already in place
- how ready you feel to make those changes and what could we do to help?
Alongside you, we will:
- take time to get to know you and build a trusting relationship
- listen and talk to you about what would make your life better and what it would take to get there
- help you increase your confidence to deal with life's challenges
- help you identify your skills, talents and passions
- help you feel heard. For example, to speak to organisations and professionals
- support you to meet new people or reconnect with others
- help you become more involved in and contribute to your community
- support you and your family/carers to get the information and advice they need
What we expect from you
We expect you to have:
- a willing and open mind
- an enthusiasm to look at things differently
- the ability for you to take the lead in achieving your own goals
- the understanding that we will not do things for you, but build your confidence and encourage you to be independent and do things for yourself
- honest communication and provide all relevant information we need to help you
To keep to planned appointments and understand that help may come to an end if appointments are not appropriately maintained.

Upon receipt of your form we will consider your information and be in touch with you to provide one or more of the following:
- information and advice
- referral to a more appropriate agency
- offered a face-to-face contact appointment
- offered a telephone conversation
