We propose to improve the way that lifestyle and wellbeing support is offered and provided to people living in Derbyshire. 

We're redesigning the provision of current services, to develop a more joined-up Derbyshire-wide approach, which offers people relevant information and access to local services.

For you it will mean that services including stop smoking, weight management and physical activity will continue to be offered, but in a more joined-up and integrated way.

We value your opinion and would like you to take part in our online survey.

This online questionnaire will link with three strands of the consultation:

  • Road shows for the public
  • Service user questionnaire
  • Professional including existing service providers
